![]() 2 However, it has become clear that smoldering multiple myeloma is in fact a heterogenous group some patients experience smoldering multiple myeloma that behaves more like MGUS, and some are destined to develop multiple myeloma requiring treatment in a short period of time (so-called high-risk smoldering multiple myeloma). 9 Long-term follow-up data of patients with smoldering multiple myeloma have suggested that the risk of progression to multiple myeloma requiring treatment is 10% per year for the first 5 years, then 3% per year for the next 5 years, and, finally, 1% per year for the next 10 years. ![]() The definition has since evolved formally into patients who have no evidence of myeloma-defining events or amyloidosis and (1) a serum M-spike of 3 g/dL or greater, (2) urinary monoclonal protein of 500 mg or greater per 24 hours, and/or (3) bone marrow plasmacytosis between 10% and 59%. The term “smoldering multiple myeloma” was created by Kyle and Greipp 1 in 1980 to classify a group of patients with evidence of a clear monoclonal gammopathy of reasonably high burden but for whom “treatment should be withheld” given the lack of myeloma-defining events. He is diagnosed with high-risk smoldering multiple myeloma based on an M-spike greater than 2 g/dL and a kappa/lambda ratio greater than 20. He has no other myeloma-defining events based on imaging and blood test results. ![]() An evaluation of the patient’s neuropathy reveals findings suggestive of diabetic neuropathy. In the first of three scenarios, the patient is found to have an immunoglobulin (Ig) G kappa monoclonal gammopathy measuring 2.2 g/dL, serum free light chains reveal a kappa/lambda ratio of 24, and a bone marrow biopsy reveals 15% kappa-restricted plasmacytosis. 1 ), 4 – 8 making the approach to the diagnosis and management of these conditions all the more challenging. 3 What this means for the practicing clinician is that any given monoclonal gammopathy can have many different fates ( Fig. 2 In a recent, prospective, population-wide screening study of the adult Icelandic population (older than age 40), MGUS was identified in 4.9% of the population at baseline, with the prevalence increasing with age 1% of patients have thus far been diagnosed with a lymphoproliferative disorder, including multiple myeloma, light-chain (AL) amyloidosis, chronic lymphocytic leukemia, and Waldenström macroglobulinemia (WM). It is estimated that the risk of progression from MGUS to multiple myeloma is approximately 1% per year. 1 The majority of MGUS cases arise from a malignant plasma cell clone in the bone marrow that does not reach the diagnostic threshold of multiple myeloma. Until recently, the incidence of monoclonal gammopathy of undetermined significance (MGUS) was based on studies from Olmsted County, Minnesota, and from Scandinavian health records, indicating an incidence of approximately 1%. Waldenström macroglobulinemia is an important cause of IgM monoclonal gammopathy, which can be associated with a wide variety of clinical features recently developed targeted therapies can achieve durable responses.Ī monoclonal gammopathy is a laboratory-based finding that is being increasingly recognized in the aging population. Light-chain amyloidosis incidence is paramount given that delayed diagnosis hampers treatment success and may result in irreversible organ damage.Īccurate typing of amyloidosis is important in distinguishing between light-chain, transthyretin, and other forms of amyloidosis. Smoldering multiple myeloma is a clinical entity that does not require immediate treatment, but selected patients with high-risk disease require more frequent follow-up. Monoclonal gammopathies represent a broad differential diagnosis and require a thorough evaluation for clinical significance. In this review, we focus on treatment strategies for patients with monoclonal gammopathies that are not multiple myeloma, including smoldering multiple myeloma, light-chain amyloidosis, and Waldenström macroglobulinemia. Recognition has become all the more important as we understand how to triage the 4% to 9% of patients with monoclonal gammopathies depending on age, with the goal of limiting overdiagnosis and misdiagnosis. Early recognition of monoclonal gammopathies along with a careful workup are essential in determining the next steps in the care of a given patient. This change has created a challenge for patients and clinicians, as a monoclonal gammopathy may be a harbinger not of multiple myeloma but of other lymphoproliferative disorders such as light-chain amyloidosis and Waldenström macroglobulinemia. Once accepted as a possible precursor condition to multiple myeloma, monoclonal gammopathies as an entity are now associated with many renal, neurologic, and dermatologic disorders of clinical significance. Our knowledge of monoclonal gammopathies is continuously evolving. Instead, you may have to rely on another technique, such as focus stacking, which is very cumbersome and doesn’t really work if you have anything moving in the frame. But at such narrow apertures, you’re bound to get a significant loss of sharpness due to diffraction.Īnd even f/16 might not cut it. To achieve such an effect, you’d need to dial in an aperture of at least f/16. You’d need an incredibly deep depth of field–that stretched all the way from the flowers in the foreground to the mountains in the background. There are flowers in the foreground and mountains in the background.īut if you were shooting with a standard lens, one that offered a standard plane of focus (that sits parallel to the image sensor), this would be really hard to do. Landscape photographers love tilt-shift lenses.īecause a tilt-shift lens allows you to maximize your depth of field–and keep the whole image sharp, from front to back.įor instance, imagine you’re shooting a sweeping landscape scene. Instead, when you photograph a building from below (while using the shift function) it’s as if you shifted the lens upward, so that your camera is levitating toward the middle of the building.īut why is this actually important? How can tilt-shift lenses help your photography? A Tilt-Shift Lens Maximizes Depth of Field You see, you can actually shift a tilt-shift lens, so that you don’t get the normal perspective distortion at all. This is referred to as perspective distortion, and it’s the reason why buildings appear to be falling backward when they’re shot from below.īut a tilt-shift lens does away with all the standard perspective distortion rules. The sides of the building will appear to converge as they go higher and higher into the sky. If you photograph the top of a building from down on the ground, you’ll end up with an interesting result: Now, technically speaking, this focusing feature is the tilt aspect of tilt-shift lenses.īut tilt-shift lenses offer another feature, too: …as are parts of the scene that are farther away.Īnd other parts of the scene will be out-of-focus, even if they’re located right next to parts of the scene that are in focus. The practical result is that parts of the scene that are near to you are in focus… This ensures that the lens glass is not parallel with the sensor.Īnd it re-orients the plane of focus in the image. In other words, the part of the image that is sharp appears like a rectangular cut-out.īut with a tilt-shift lens, you have the option of tilting the lens. Well, the parallel glass is designed to create a parallel plane of focus. ![]() This subsequently has created an incredibly easy "tilt-shift" lens system to use, even if we're using that term loosely. So the glass inside the lens is parallel to the image sensor. There are focus and aperture rings to adjust to switch between macro and infinite focus, the outer part of the optic can be pulled up (macro) or pushed down (infinite). Instead, this is "tilt shift" made simple. Since it can only achieve the selective focus effect of tilt-shift lenses, there are no knobs or swings. ![]() Lensbaby's Edge 80 optic takes a different approach. Canon's lens is an example of fine craftsmanship, with the ability to tilt plus or minus 8.5 degrees, shift plus or minus 12 millimeters and not one but two swings to bring new dimensions by changing the axis. It's probably more accurate to call the Edge 80 a selective-focus optic. Still, compared with a Canon TS-E 24mm f/3.5L, a loaner from Canon for the purpose of comparing a real tilt-shift lens to this Lensbaby optic, the Edge 80 is much easier to use - and that's saying something given the nature of Lensbaby's products. It neither tilts nor shifts like typical lenses (sorry, architectural photographers), but it can change the plane of focus with its 12-blade adjustable aperture. ![]() These sheet music are mostly traditional Celtic music, but some are world music, Browse Alfred Music piano methods, supplementary piano music, sheet music, standard masterworks, and more from leading authors, educators, and pianoscale. The case has a prominent curve that is not immediately noticeable and this allows the watch to sit very comfortably on the wrist. The main case is both brushed and polished but look closer to see that the aggressively knurled bezel edge is bead blasted. When you go 360 with the Avalon you will notice the 3 different case finishes used, and the attention to detail with all of them. Lume junkies rejoice, this is the real deal. The Avalon glows as well as any watch at the top of the lume mountain, and maybe more. If you are still not convinced as to why you would want that X1 lume, take a look below. With this being a dive watch that is meant to be used, I don’t think anyone will really be complaining about that. While some may prefer the use of a whiter lume compound, the Nodus Avalon uses X-1 C3 Superluminova, so it is somewhat of a trade-off. One thing that you will notice in my photos is the green color of the lume paint. Its dial looks matte white but does have a very subtle sheen which plays well off the diamond cut hands and applied markers. The dial is crisp and uncluttered, allowing for easy readability at a glance. The Avalon, a 300 Meter dive watch, has so much going for it, it is hard to know where to start. At first glance, it may seem similar to a Doxa or Eterna Super Kontiki case but when you look at the profile, you can see this watch has its own personality. If you have not watched the video yet, please do so, but if you like photos, make sure to look at all attached in this review, as you really need to look at all sides of the watch to see all it offers. The Trieste was a decent microbrand offering, the Avalon might be setting a new standard. ![]() Writing this today, it is amazing how far the brand has come. I had to go back and read my original review and while I did like the watch I had a few issues but was hopeful for the brand. It has been a while since I reviewed the Trieste from Nodus and I was not able to get my hands on their second or third models, though I do hope to get a Contrail in hand soon. The young owners, Wes and Cullen, are not guys to give up though and pushed forward to make the Avalon a reality. I have been looking forward to this model for almost two years now, as this was meant to be the second watch in the Nodus stable, but a few setbacks kept that from happening. It does not store any personal data. The Nodus Avalon is the fourth watch to be produced from the Los Angeles based brand and while they openly admit that production is outsourced assembly and QC is done in-house. The information provided here is not investment, tax, or financial advice. I expect to see managed funds available to the investor, which could help manage some of the diversity and risk issues. Do your homework before buying an NFT, and if you aren't sure which one to buy or invest in, hang on for a while as the space matures. While these opportunities are exciting, investors should keep in mind there is a risk of picking the right NFT, ensuring the transaction is secure and understanding the underlying value of the asset itself. For those investors looking for exposure to the asset class, I believe that many opportunities will come our way, whether through managed funds or individual NFTs themselves. ![]() ![]() NFTs are, in a sense, just another asset class supporting the currency asset and blockchain exchange growth. Finally, anything that looks, smells or feels like part of the crypto or digital revolution has taken our society by storm. Also, as blockchain use becomes a more popular way to have proof of ownership, NFTs are likely to keep coming. Whereas paintings, baseball cards and other such works can be easily copied, NFTs provide true sole ownership, which is appealing to many. The main reason why NFTs are likely to stick around and grow in popularity centers around the idea of uniqueness - having access to something that no one, in theory, could ever have access to without the owner's active decision to replicate. Time will tell, but with investments and use cases in the space booming and buyers clamoring for access, I believe we can expect to see NFT growth in the coming years. It can be used to purchase NFTs in part or full in some cases, and as the value of the assets in Ethereum grew, so did the buying power of the underlying owner. For example, Ethereum surged over the past year. I believe part of the boom also came around the growth of crypto. Covid-19 drove a boom, with the NFT market reaching a valuation of more than $250 million in 2020, according to the NFT Report 2020 (via the Times). So what is propelling the latest rise in popularity? I believe one reason is the coronavirus. They’ve been around since the mid-2010s, according to the New York Times, but their popularity seems to have surged recently. You can then trade it and financialize it - like a new way to trade sports cards. You might not get rights to the IP to resell, but you do get the unique bragging rights of owning that clip. The NBA is also investing in projects to sell NFTs and will be selling clips in NFT form. The benefit to the band is if Kings of Leon sells an NFT to one owner and limits the digital reproduction, they can receive royalties and control the distribution of their product. For example, in March, the band Kings of Leon sold unreleased music via an NFT. First, selling an NFT can offer significant benefits to the artist. Like is like, and every token of equal value can be exchanged for another. With cryptocurrency, the value and transparency are more obvious you can exchange one Bitcoin for another, for example. With NFTs, each token has a unique value and cannot be exchanged for another of equal value. The similarities between crypto and NFTs are that they both have a stored digital record on a blockchain. A common misconception is that NFTs are a type of cryptocurrency. Hitfilm Express (Hitfilm EXPRESS is the free version, Hitfilm is the paid version)
